Serious Incident Response Scheme (SIRS) Agreement Form


SIRS was extended to include homecare services from 1 December 2022. 

It applies to staff, volunteers and contractors who have obligations to report all incidents that occur in connection with the provision of care. 

This includes any act, omission, event or circumstance that occurs, or is alleged to haveoccurred or is suspected to have occurred in connection with the provision of care and services to a consumer that has or could reasonably expected to have caused harm to the consumer or another person.

The following are considered reportable incidents: 

  • Unreasonable use of forc

  • Unlawful sexual contact or inappropriate sexual conduct

  • Psychological or emotional abuse

  • Unexplained absence from care or missing consumer

  • Stealing or financial coercion by a staff member

  • Neglect

  • Inappropriate use of restrictive practices

  • Unexpected death.

All incidents and near misses must be reported to Westmont.

You must report the following priority incidents to Westmont within 24 hours:

  1. Reportable incident caused or could reasonably cause a consumer physical or psychological injury or discomfort that requires medical or psychological treatment.

  2. Where there is reasonable ground to report the incident to the Police.

  3. Unlawful sexual contact or inappropriate sexual conduct inflicted on a consumer.

  4. That is an unexpected death of a consumer.

  5. Where a consumer goes missing in the course of providing homecare services.
